About the Company
Casco Bay Community Support Services’ Skills Development program assists participants with increasing their independence by learning the skills necessary to access community resources, including connecting with natural supports needed to achieve their particular goals.
Based on needs identified during a Comprehensive Assessment, the person receiving services collaborates with the provider in the development of an Individual Treatment Plan (ITP). The ITP outlines specific goals that the client would like to achieve and how staff and natural supports will support the client in learning new skills allowing them to reach their short and long term goals. Training will be provided in independent living skills, such as how to use public transportation, how to budget, how to access 12-step programs, and how to select and participate in educational, vocational, and social activities.
Through partnering with their Case Manager, participants will begin to overcome barriers and connect with community resources to live a healthier and more personally rewarding life. The Case Manger works with participants to assess their needs including medical, social, residential, educational, vocational, and emotional. Once need areas are identified, the case manager and client will create an individualized treatment plan (ITP) outlining goals and objectives to work toward to address needs.
Through this process, clients can begin taking steps toward becoming more independent and achieve wellness. The participant’s Case Manager will provide information and consultation to the client, and their immediate support system, to assist them in overcoming barriers and achieving identified goals. Case Managers may support the client’s efforts in many different ways including making referrals to other relevant service providers, linking them to available community resources, helping to identify stressors and encouraging the use of coping strategies, working together to improve communication and self-advocacy skills, engaging in problem solving, connecting with natural supports, employment and volunteer opportunities.
